Our Story

Who are we?

The name, Te Awakairangi, means ‘esteemed’ or ‘precious’ and was originally given to the Hutt River in the time of the Ngai Tara settlers.  It was bestowed on the Network by one of our founding Trustees, Kara Puketapu, of Te Rūnanganui o Te Āti Awa, when we were established in 2012.

Te Awakairangi Health Network is a primary care network that provides quality primary care services across the Hutt Valley. We work with the general practices and a wide range of healthcare providers and community organisations to empower people and enable them to receive the care they need.  More about what we do can be found below.

What We Do

11000
People in the Hutt Valley

We serve the population of the Hutt Valley through a wide range of services and programmes, under contract to Hutt Valley DHB.

100000
Enrolled Patients

Almost 115,000 people are enrolled with Te Awakairangi general practices. Of these people, 18% are Māori, 9% are Pacific, and 14% are Asian. Around 23,000 (20%) live in areas of high deprivation (Quintile 5).

10
General Practices

Our 17 general practices provide primary care services from 19 clinic sites within the Hutt Valley.

Primary Care supports you by...

Primary care support you by having expert and committed general practice teams who:

  • Know you and your whānau and what matters to you
  • Help you to understand how to stay healthy
  • Support you to manage your health conditions and concerns
  • Are accessible and convenient
  • Provide comprehensive care over time
  • Connect you with other health and social services, as needed

We support the general practice teams to support you and your whānau by providing services and programmes that extend the care they can give you.

We also support the general practice teams to provide more intensive support and intervention for you and your whānau when it is needed. We provide this support through our dedicated community-based teams including outreach nurses, community health workers and social workers and our practice-based teams including clinical pharmacists, health improvement practitioners, health coaches and primary mental health professionals.  Click here to see more about how we can help you.

We collaborate with whānau, iwi, community, health and social services providers to achieve the vision of everyone in the Hutt Valley being healthy and well
