The Outreach Nursing Service works alongside General Practices to provide assistance to eligible patients with high health needs, who may experience barriers to accessing primary health care or other patients who are difficult to engage.
Outreach Nurses offer a holistic service which includes health and other issues that can impact on a patient’s general wellbeing.
Support can be given for health education, management of conditions and advocacy with other agencies.
The nurses also work with a patient’s family and whānau to achieve agreed outcomes.
They work across all General Practices in the Hutt Valley, except Whai Oranga and HUCHS who have their own SIA funded nurses.
The team consists of a Team Leader and 3 Registered Nurses who work in the community, visit patient’s homes, workplaces or other venues convenient for the patient.
Referrals are via General Practice doctors and nurses.
