Mō te tūnga | The Role
We have an exciting opportunity for a Health Coach to join our Access and Choice team and be part of an exciting new behavioural health model of service delivery.
As part of a Greater Wellington Wellbeing Collaborative, we have implemented a new model of service delivery which places health coaches within a general practice as part of the practice team.
As the Health Coach you will work alongside Health Improvement Practitioners, Support Workers, and the General Practice team to partner with patients experiencing issues that impact on their health and wellbeing.
In this role you will support patients and identify their own priorities and gain the knowledge, skills, tools, and confidence to become active participants in their care so that they can reach their self-identified goals. You will work collaboratively to develop active partnerships with patients motivating them to make positive lifestyle changes, reduce high-risk behaviours, manage stress, and improved their health and wellbeing.
The new service model will give you the satisfaction of knowing you have been able to support and empower people to achieve healthy lifestyles through sustainable change, and are receiving the care they need.
We can offer full training and a comprehensive orientation, regular supervision, and professional development as well as the opportunity to be part of a new team working to support the needs of the Hutt Valley population.
Mōu | About you
- Recognised Health Coach training or willingness and ability to complete.
- Experience working in primary care directly with patients as part of the health care team.
- Experience working with Māori, Pasifika, and Youth.
- Ability to demonstrate your commitment to the principles of the Treaty of Waitangi and promoting positive outcomes for Tangata Whenua.
