Health Improvement Practitioner – General

Part of the Access and Choice, Integrated Mental Health and Addictions wellbeing initiative based in general practice

We are looking for a registered health professional to take up the role of Health Improvement Practitioner.

• Progressive Primary Health Network

• Hutt Valley based

• Part time or full time opportunity for the right candidate

Te Awakairangi Health Network (TeAHN) is a dynamic and innovative organisation that plans, funds and provides a wide range of primary healthcare services to people living in the Hutt Valley.

The role of the Health Improvement Practitioner is an integral part of the general practice team, working directly with individuals and whanau to help improve their mental health and wellbeing. This includes providing consultations and advice to other members of the general practice team.

We are looking for a registered health professional who is passionate about people and health to take up a dynamic position within the Access and Choice team.  The role will be based in the Hutt Valley area and the successful candidate will have a passion and drive to work in this space with confidence.

The successful candidate needs to be self-motivated, organised and a great communicator with the ability to build rapport easily and work accurately independently.
