Registered Nurse – Outreach Services Hutt Valley

  • Working in a team of registered nurses and registered social workers
  • Progressive Primary Health Network
  • Hutt Valley based
  • Full time (although hours can be negotiated for the right candidate).
  • Ideal for someone with previous primary health care experience.
  • 5 weeks Annual leave per annum
  • Full and Part -time options available

Te Awakairangi Health Network (TeAHN) is a dynamic and innovative organisation that plans, funds and provides a wide range of primary healthcare services to people living in the Hutt Valley.

Healthcare provision is moving closer to home to provide care for clients in the communities where they live in a way that suits their needs.

Te Awakairangi Health Network is a primary care network focussed on achieving equity and improving the quality of our health services for whānau living in the Hutt Valley. We work with general practices and community service providers to promote and empower people to receive the healthcare services they need and deserve.

Mō te tūnga | The Role

We have an exciting opportunity for a Registered Nurse to join our Outreach team.

Our Outreach Services team work to ensure that we are reaching those populations most in need and support whanau to connect with primary healthcare services. The nursing role has a strong focus on health literacy and health education in regards to long term conditions.  The team consists of registered nurses and social workers and work directly with general practices to support their enrolled populations.  The team also work with other health providers in the Hutt Valley to support clients to access the healthcare that they need.

If you are enthusiastic, self-motivated and a great communicator with strong clinical skills then we would like to hear from you.

The successful applicant will be a registered health professional with knowledge and experience of working with Māori and Pacific communities in primary care.  Excellent organisation and time management skills with an ability to work independently is essential.
