Comprehensive Primary Care Team Implementation Lead

Location: Kokiri Marae Seaview Lower Hutt

Role Type: Fixed Term

Salary: Negotiable

Hours: Between 30 – 40 hrs per week

Job Position:  Fixed term funding has been provided to strengthen primary and community care through the establishment of CPCTs (Comprehensive Primary and Community Care Teams) within local provider networks. These teams combine the primary care services provided by general practitioners, nurse practitioners and practice nurses with new roles such as physiotherapists, practice pharmacists, care coordinators, and kaiāwhina. The new roles will be integrated into extended primary and community care teams, largely in general practice settings.

Our group of providers[1] is working together to implement CPCT in the Hutt Valley and we are collectively looking for a highly skilled person to support our implementation.  We are looking for someone who has knowledge of the primary care health sector, change management experience and data reporting experiences and skills.

Ideal Person:

  • In-depth knowledge of the New Zealand healthcare system, including Hauora Maori organisations, PHOs, general practices and other primary and community

Maori organisations, PHOs, general practices and other primary and community care organisations.

  • Has (or can develop) trusted relationships with mana whenua and with primary and community providers, including general practices.
  • Understands the application Te Tiriti o Waitangi and WAI2575 to primary care.
  • Strong data analysis and reporting skills.
  • Exceptional interpersonal, communication, and facilitation skills.
  • Strong project management and change management skills.
  • A tertiary/ higher education qualification in a discipline relevant to the role.
  • A full and current driver’s license
